Background: Postpartum hypertension affects 1 in 10 deliveries. Telemedicine may improve blood pressure monitoring but evidence from randomized trials is limited. Methods: We conducted a multicenter, open-label trial comparing telemedicine-based home blood pressure monitoring with standard clinic follow-up in 600 postpartum women with hypertensive disorders. Results: The telemedicine group achieved 95% adherence to monitoring vs. 60% in the control group (p<0.001). Mean systolic blood pressure was 3.2 mmHg lower at 6 weeks (p=0.01). Conclusions: Telemedicine enhances monitoring adherence and modestly improves blood pressure control. Registration: ClinicalTrials.gov NCT04567890.
Hypertensive disorders are the leading cause of maternal morbidity. Telemedicine offers a promising avenue for remote monitoring, yet prior studies have been observational. This trial provides the first randomized evidence on the effectiveness of a telemedicine-based approach in the early postpartum period.
We enrolled women aged 18–45 with a diagnosis of preeclampsia or gestational hypertension. Participants were randomized 1:1 to either a telemedicine group (home BP monitor and smartphone app) or standard care (clinic visits at 1 and 6 weeks). The primary outcome was adherence to blood pressure monitoring at 6 weeks. Secondary outcomes included mean systolic BP and hospital readmission rates.
