Customer feedback survey: ask clearly, act on the answers

A customer feedback survey asks about one recent experience, measures it with a consistent scale, and gives the customer room to explain what happened. Start with overall satisfaction, separate service quality from communication, and finish with one optional improvement question. Keep contact details optional so giving feedback does not depend on agreeing to a follow-up.

Question bank

Questions that lead to a useful next step

Overall, how satisfied were you with your recent experience?

Very dissatisfied / Dissatisfied / Neither satisfied nor dissatisfied / Satisfied / Very satisfied

Provides a consistent overall measure. Keep the same wording and response order when comparing survey periods.

How would you rate the quality of the service you received?

Very poor / Poor / Fair / Good / Very good / Not applicable

Separates the work itself from surrounding communication. Not applicable avoids forcing a rating from someone who did not receive the service.

How clear was our communication about what would happen next?

Very unclear / Unclear / Neither clear nor unclear / Clear / Very clear / Not applicable

Tests expectations and next steps without assuming the customer had a problem.

What is the one thing we could improve about this experience?

Optional open response

A single focused prompt is easier to answer and sort than a request for every possible suggestion.

Read the results

Read the pattern before choosing a fix

  1. Show the denominator alongside each result

Report how many people answered each question, not just a percentage or average. Optional questions can have different response counts. Keep Not applicable separate from the scale and state how you handled it.

  2. Compare like experiences

Compare the same service, survey wording, and invitation method over time. A change in who receives the survey can change the result even when the service stays the same. Respondents describe their experiences; they may not represent every customer.

  3. Connect ratings to comments

If service quality is positive but next-step clarity is poor, look for comments about updates, arrival windows, or handoffs. Group comments by theme, preserve the customer's meaning, and choose an owner for the most actionable issue. Treat this as a clue to investigate rather than proof of its cause.

  4. Close the loop with a specific change

Record what you will change, who will do it, and when you will check again. For example, revise the appointment confirmation to include an arrival window, then compare the same communication question in a later survey. Follow up with an individual only when they requested contact.

Mistakes to avoid

Small wording choices that distort feedback

Asking a leading question

Replace How excellent was our service? with How would you rate the quality of the service you received? A neutral question lets a negative experience fit the answer scale.

Combining two experiences in one rating

Was our team friendly and efficient? cannot tell you which part needs work. Ask about one dimension at a time and keep only dimensions you can act on.

Making every explanation mandatory

Require the core satisfaction question and leave comments optional. Someone can give a useful rating without writing an essay or sharing contact information.

Treating a low score as a complete diagnosis

Read the explanation and investigate the experience before choosing a fix. Avoid promising that every suggestion will be implemented.

Frequently asked questions

What should a customer feedback survey include?

Include one overall satisfaction question, a small number of questions about aspects you can improve, and an optional open response. Identify the experience being rated in the introduction. Ask for contact details separately and only when a follow-up is useful.

How long should a customer feedback survey be?

Use the fewest questions needed for a decision. The blueprint here focuses on three ratings and one comment, with optional follow-up fields. Read it on a phone and remove any question whose answer has no clear use.

Is customer feedback the same as an NPS survey?

No. A feedback survey can examine satisfaction and specific parts of a recent experience. An NPS survey centers on a likelihood-to-recommend question. Choose the measure that matches the decision you need to make; keep their scoring methods separate.

Should customers have to give their names?

A name is usually unnecessary for understanding a service pattern. Keep identification optional unless you have a clear reason to request it, and explain that reason. Do not describe a survey as anonymous if you collect identifying information.

How to build it

  1. Choose a category and a form type in the wizard. Answer three Atty Suggestions questions, then give the form a title. Atty builds the fields in the background. Edit the fields and ask Atty to refine them, then publish and share the /f/ link.
