Contact us form: a short intake for general questions

A contact us form collects a visitor's name, a way to reply and their question, so the right person on your team can answer it. It works best when it asks only for what a reply needs, and when the topic list is short enough for the team to sort by at a glance. The judgment is how much to ask before you know the question, since every extra field can lower the number of people who finish.

Questions to leave out

Questions a general contact form should not ask

What is your annual revenue or budget?

Puts a sales qualification on a general question. If you need a budget for a quote, use a request form that says so at the top.

Where did you hear about us?

Useful for marketing reports, but it does not help answer the question. If you need it, ask on a separate, optional line and say why.

Would you like to join our newsletter?

A marketing choice mixed into a support question makes that consent unclear. Keep newsletter sign-up on its own form.

Please give your full home address.

A general inquiry rarely needs a home address. A reply by email is enough for most of them.

Field blueprint

The fields a reply depends on

FieldTypeRequiredWhy you ask it

Name

Short textRequired

Lets the reply open with the visitor's name, which reads far more personally than a generic greeting.

Email address

EmailRequired

The reply route. A general question with no way to answer it cannot be closed.

Phone number (optional)

Only if you would like a call.

PhoneOptional

A second route for visitors who prefer to talk, without making it the default.

Company or organization

Short textOptional

Helps when the answer depends on the visitor's type of business. Keep it optional.

What is your question about?

Our products or servicesAn existing order or accountPricing or availabilityPartnerships or pressSomething else
DropdownRequired

Gives the team a sort before anyone opens the message, so the right person sees it first.

Your message

A few sentences is enough. Include a product or service name if you know it.

Long textRequired

The question itself. Ask for the question rather than the whole background story.

Helps the team order its queue. It is not a guaranteed response time.

Sorting and answering the messages

  1. Check the topic before you read the message

Use the topic field to decide which message to read first, and let your own team decide who answers each topic. The form does not route messages to people for you.

  2. Set the reply window in the confirmation

Saved messages are processed on the hourly production schedule, and only the form's creator is notified after that. Write the confirmation around a reply window your team can keep, and do not promise a quick answer.

  3. Use the grid for the weekly queue

Messages appear in a grid, and a CSV export lets you count topics by week. A topic that grows fastest often points to a page on your site that needs rewriting.

Frequently asked questions

What should a contact us form ask for?

A name, an email address to reply to, a topic the team can sort by, and the message itself. Phone and company are useful only when the reply depends on them. Anything else adds friction without making the reply better.

Can I put a contact form on my website?

Link your contact page to the form's public address instead. Attain OS does not place a form inside another website, so the link is the route your visitors use to reach it.

Is there a contact us template I can copy?

Use the field list in this guide as the template, and keep the topic options short. Rename the labels to match how your team describes its work, and remove any question you do not sort or answer by.

Do I need a phone number on a contact form?

Usually not. An email address is enough to reply. Keep the phone field optional, and ask for it only where your team would call the visitor.

How to build it

  1. Choose a category and a form type in the wizard. Answer three Atty Suggestions questions, then give the form a title. Atty builds the fields in the background. Edit the fields and ask Atty to refine them, then publish and share the /f/ link.
