Tell Atty what you need to ask and get a working draft. Share a public link that needs no login, and let each submission open a support case for your team.
Most form tools stop at collecting answers. This online form builder is made for small teams whose answers turn into work: a repair request, a support question, a volunteer sign-up. You describe the form in plain language, Atty drafts the fields, and you refine it the same way. The public link needs no account from the person answering. If you want, each submission opens a support case with a priority and tags, so the request lands where your team already works. It is deliberately simple: there are no file uploads, no branching logic and no payments.

Header, short text, long text, number, email, phone, date, radio, checkbox, dropdown and rating.
Anyone with the link can answer, and a spam check and terms acceptance protect the form.
Turn it on and every submission opens a case with the priority and tags you choose.
Review answers in one table and filter them, with optional name, email and phone capture.
It fits small teams that want answers to land as cases or records: service requests, support questions, intake forms, sign-ups and simple surveys. You get a public link, a table of submissions you can filter, and the option to open a case for each one. It does not fit every job. If you need file uploads, branching between questions, payments, response charts, a CSV export or a form embedded in your own website, this is not the right tool today. Forms are shared as a link, and answers are reviewed in the table inside the app.
In the form settings you can switch on case creation, choose a priority and add tags. Each submission then becomes a support case your team can assign, work and close. When the respondent gave an email address, a customer record is created for them too; without an email, only the case is created. Submissions are processed on a schedule, hourly in production, so the case appears shortly after the form is sent, not at the same second. Your team then works it like any other case, with statuses, notes and assignees.
If you are weighing this against Typeform, read our Typeform alternative page for an honest side-by-side. In short, Typeform has logic jumps, payments, embeds and response charts, and Attain OS has none of those. What Attain OS offers is the step after the answer: a case or record in the same workspace as your customers, bookings and tasks. Attain OS does not import forms or responses from other tools, so you describe the form again and share the new link.
The owner describes a repair request: name, email, phone, type of equipment, urgency and a description of the problem. Atty drafts it and the owner marks two fields required. With case creation on, every request opens a support case with a set priority and a web-form tag, and the technician who is assigned sees it in the case list. The email on the form links the case to a customer record.
A volunteer coordinator builds a sign-up form with a dropdown for the day, a rating for experience and a short comment box. No case creation is needed here. Answers accumulate in the submissions table, where the coordinator filters by day before each event. Follow-up is by hand from that list; the product does not send bulk messages.
In the Forms app, say what the form is for and who will fill it in. Atty drafts the fields for you to review.
Ask for changes the way you would ask a colleague: rename a question, make it required, add an option. You can also edit fields by hand.
Copy the public link and put it wherever your audience is. Respondents need no account.
Optionally open a support case for each submission. Signed-in, Atty can read a form and draft a task or case for a person to approve, but it cannot edit the form.
A close-to-finished draft — not a blank page. Here's the kind of result the flow hands you, ready to review, tweak, and publish.

Only if you switch on case creation for that form. A customer record is created only when case creation is on and the respondent left an email.
No to all three. Forms have 11 field types and a public link, nothing more.
