Introduction
Submit this request before any draft goes to the other party. Give the reviewers the facts they need to start, and paste a link to the current draft.
A contract approval form tells the review team what the contract is, who the other party is, what it is worth, and how far it departs from your standard template. Reviewers can triage a request quickly when those facts are complete, and they have to ask for more detail when they are not. The form records the request and where the draft is kept; it does not decide whether the contract is approved.

Submit this request before any draft goes to the other party. Give the reviewers the facts they need to start, and paste a link to the current draft.
Thanks. Your contract request is recorded, and the review team will reply through its usual process. Do not send the draft to the other party until you have a reply.
Use the total you expect to receive or pay over one year. Write 0 only if the contract has no value.
Paste a link your reviewers can open. This form does not take the draft itself.
Name each change in plain words, such as a new liability cap or a different termination notice.
| Field | Type | Required | Why you ask it |
|---|---|---|---|
Requester name | Short text | Required | Tells reviewers who can explain the business context of the contract. |
Department | Short text | Required | Shows which team owns the relationship and the budget behind it. |
Other party's legal name | Short text | Required | Matches the contract to the right vendor or customer record, using the name that will appear in the signed document. |
Contract typeVendor or service agreementCustomer or sales agreementPartnership or reseller agreementContractor or employment agreementNon-disclosure agreementLease or property agreementOther | Dropdown | Required | Helps the team choose the reviewer who handles that kind of contract. |
Estimated annual value (US dollars) | Number | Required | Helps set the review level, such as whether a senior approver must sign off. |
Requested start date | Date | Required | The earliest date the contract should take effect. |
Term lengthFor example: 12 months, renewing each year unless either party cancels. | Short text | Required | Shows how long the commitment runs and whether it renews on its own. |

Personal data involved?YesNoNot sure | Single choice | Required | Flags whether a privacy review is needed before anyone signs. |
Where the draft is keptPaste a link your reviewers can open. | Short text | Required | Tells reviewers where to find the current version. This form does not take the draft itself. |
Signing deadline | Date | Required | The day the contract has to be signed. Review takes time, so the deadline should leave room for it. |
Expected signer | Short text | Optional | Names the person who will sign, since that can change the approval path. |
Submissions are stored straight away and processed on an hourly run that notifies the form's creator. Only the creator is notified, so agree before the form goes live how everyone else on the review team hears about a request.
The form does not send the request to legal or to the expected signer. Route it through the process your team uses, and record the decision where your team keeps its approvals.
A request can also create a Customer Care case, with a priority, tags, and assignees set in the form settings. Turn that on only if the review team wants every request tracked that way.
Without a value, reviewers cannot tell whether a senior approver must sign. Ask for an estimate, and accept 0 only when the contract has no monetary value.
A request that notes changes but does not name them sends reviewers back through the whole draft. Name each change in one line.
A signing deadline on the day of the request leaves no time for questions. Submit early enough that a question can be answered before the deadline.
A submitted form records a request. The approval decision happens in your review process, and the requester should wait for it before the draft goes out.
Here you get the fields to build from, not a template file. Attain OS forms are shared by link and are not exported as Word or PDF files. Use the fields above, and set the approval levels in your own contract policy.
No. The form gathers the facts a reviewer needs to start. Legal decides what the contract says, and your counsel should set the thresholds that decide who signs.
The form does not take the draft itself. Keep the draft in the document system your team already uses, and paste a link to the current version in the location field.
Only the form's creator is notified, and only after the hourly processing. Decide before the form is shared who else needs to read requests, and how they will hear about each one.
Choose a category and a form type in the wizard. Answer three Atty Suggestions questions, then give the form a title. Atty builds the fields in the background. Edit the fields and ask Atty to refine them, then publish and share the /f/ link.
