Policy acknowledgement form: record receipt, questions, and the version

A policy acknowledgement form records that a named person received a specific version of a policy and had a chance to ask questions. It does not show that the person understood or agreed to every term, and whether it needs more wording is a question for your HR advisor or counsel. Name the policy and its version in every form, and send the document through your usual channel rather than relying on the form to deliver it.

What this agreement means

What the employee confirms, in plain words

  • The person received the policy named on the form, at the version named there.
  • The person had a chance to read it and to ask questions before they responded.
  • The person knows where to find the current version if the policy changes.
  • The form records a response. It does not deliver the policy, train anyone, or prove the policy was read.

This is a drafting aid, not legal advice. Have your own counsel or HR advisor review the wording, especially for conduct, safety, or handbook policies, and decide whether any statement needs to say more.

Field blueprint

Fields that tie the response to one version of one policy

FieldTypeRequiredWhy you ask it

Employee name

Short textRequired

Matches the response to the person in your HR records.

Work email

EmailRequired

Gives HR a way to follow up on questions and to match the response to the right record.

Policy title and version

Copy the title and version number from the policy document itself.

Short textRequired

Ties the acknowledgement to one document. A response to an old version does not cover the new one.

Date you received the policy

DateRequired

Shows when this version reached the employee, which matters when the policy changes.

Policy receipt

Read the statement above, then choose the answer that describes you.

Section header

Separates the document details from the statement the employee responds to.

Which statement describes you?

I received this policy and have read itI received this policy and have questionsI have not received this policy yet
Single choiceRequired

Separates a clear acknowledgement from a question or a missing copy. Treat the last two answers as open items.

Questions about the policy

Keep personal health or family details out of this box.

Long textOptional

Lets the employee raise a question without refusing the acknowledgement. Someone in HR has to read it and respond.

Date you responded

DateRequired

Records when the response was given, which can be a different day from when the policy was received.

Questions to leave out

Questions and promises to leave out of an acknowledgement

Do you agree to every term in the employee handbook?

Blanket agreement to terms a person may not have read invites an argument later. Name the specific policy and version, and ask for receipt.

Do you waive your right to bring a claim against the company?

Waiver language is a legal decision for your counsel. It does not belong in an acknowledgement drafted by HR.

What is your Social Security number and bank account?

An acknowledgement needs a name and a work email. Keep identifiers and pay details in the systems built to hold them, not in a policy response.

Do you promise to follow every future policy change?

A promise about versions that do not exist yet is not a receipt. Ask for acknowledgement of each new version when it is issued.

After a response arrives

What HR does with each response

  1. Submissions are stored, then processed hourly

Responses are stored straight away, and the hourly processing run then notifies the form's creator. The form does not send the policy itself, so send the document through your normal channel and keep a record of the version you sent.

  2. Treat not received yet as an open item

Any response saying the person has not received the policy needs a follow-up through your normal channel. Log that follow-up in your own record, because the form does not track it.

  3. Compare responses with the staff list each cycle

Export the grid as a CSV and compare it with your current staff list. Anyone who has not responded to the current version is the person to follow up with.

Frequently asked questions

Does a policy acknowledgement form need an e-signature?

Attain OS does not add e-signatures to forms. Whether a typed name and a click count as acknowledgement for your policy is a question for your HR advisor or counsel, and the answer can differ from one policy to the next.

Can I use a policy acknowledgement template instead?

A template works if it names the policy and its version, records receipt and questions, and includes a date. The fields on this page cover those points, and you can use them in any document your team already keeps. Keep the final wording approved by HR.

How should we handle a handbook acknowledgement?

Ask for acknowledgement of a specific handbook version, not the handbook in general. New hires often receive the handbook at a different point than current staff, so track those two groups separately.

How often should staff acknowledge policies?

Ask for an acknowledgement each time a policy version changes, and for policies that apply to new hires, at hire. Your HR advisor should set the rule for each policy, because the right cadence depends on the policy.

How to build it

  1. Choose a category and a form type in the wizard. Answer three Atty Suggestions questions, then give the form a title. Atty builds the fields in the background. Edit the fields and ask Atty to refine them, then publish and share the /f/ link.
